2011 Annual Meeting Forms

• Annual Meeting Proxy | If you will be unable to attend the Annual meeting have all those named on your deed fill out this form. The top portion should indicate the person you are choosing to vote in place of yourself. If you do not want to designate any one individual, simply leave the top portion blank and the vote will be automatically given to the Board President.
• Nomination Petition | If you are interested in running for a position on the Board of Directors complete this form and return it along with the Candidate Biography (below) to the HOA by Thursday February 10th.
• Candidate Biography | Fill this form out and return along with Nomination Petition.

• Summary of Changes Bullet Point Version | This is a summary of the changes that were made to the Governing Documents in 2009.
• 2009 Amended Bylaws | This document is the Ammended Bylaws from 2009.
• 2009 Amended Articles of Incorporation | This document is the Amended Articles of Incorporation from 2009.
• 2009 Amended Declaration | This document is the Ammended Declaration from 2009.
